The City of Lafayette is no longer using paper forms. We have transitioned to online filing through MUNIRevs.
Existing vendors were mailed their registration information in October 2018. If you have misplaced your letter or did not receive it, please contact the finance department at 303-665-5588 and request another copy to be emailed to you.
Vendors that do not have an active account with the City of Lafayette must register for an account using the sales tax application link below.
